Vendor Based Special Event Forms

This article will show you how to create an easy form for vendors to fill out and register for an upcoming event.

An event-based permit can be anything from a farmers market to renting a park pavilion. It is an event-based permit for a one-time event that will not be renewed. Here are some gold standards to help you make the best form possible.


Creating the event form

Step 1

  • At the bottom of the permit types tab on the left side of the screen. Click the "+ New" button.

  • Name your new permit.

Mark YES, this permit is event-based.

  • Hit save.

Step 2

  • Once you are viewing the "Permit overview page, select an answer to the question "what type of event registrations will this take"

  • If you choose "Event participant registrations, you will get the follow-up question, "do you want to allow applicants to specify new events when registering." You can click yes or no, and restrict event choices to what we already have in the system.

  • On the right-hand side of the "Permit overview page," select the department button on the left side of the screen.

  • Add the departments that will need to review applications.

Step 3

Building the form. Some fields that might be helpful to have on the form would be

  • Datepicker: this allows applicants to choose their event's date or date range.

  • Timepicker: this allows applicants to go even further and put the time of their event.

  • Map: this can allow applicants to be able to show on a map where their booth or event will be held.

Using the Calendar

Civic Review has a calendar that can be helpful in scheduling when events occur. Civic Review will place events in the calendar, but will not notify the applicant of any overlapping events.
