Where to find and edit the Signature Affidavit within a form

Find and edit the affidavit that the applicant signs

At the end of every application the applicant will have an opportunity to digitally sign their application. When they're completing their application that signature affidavit is found on the Signature Page.

The text or affidavit on this signature page is completely customizable. You can include multiple paragraphs of text and use bold or italics formatting by utilizing markdown formatting.

Locating and Editing:

To find and edit the signature affidavit, you'll first navigate to the permit settings for the specific permit you'd like to edit. Choose the permit type from the lefthand menu; then scroll down near the bottom of the permit settings until you see a field titled Application Signature Affidavit.

You can add to or edit the text within that box and then hit the "Save" button at the bottom of the screen.

Your updated affidavit will be included on all new applications.

Where to Find the Affidavit in Submitted Applications:

After an application is submitted, staff may want to see who signed the application, or when the application was signed. This information is not included in the permit details section, but can be found within the "Terms and Fees Overview" section.
